LARGE GROUP AIR REQUEST FORM

(10 OR MORE INDIVIDUALS)

All large groups (10 or more) requiring air transportation arranged by WorldStrides must complete this form.  A nonrefundable $100 per person deposit (in addition to the $100 per person group registration deposit) is required when booking.  This deposit is due after you have approved the airfare quote received from WorldStrides and you are ready to reserve your flight tickets.  Air Packages are reserved only when the flight information has been approved AND deposits are received. 

All Air Request Forms must be submitted to WorldStrides by April 23, 2010.

NOTE: You will receive an airfare quote via e-mail within approximately 3-5 business days.  This quote is only valid for 7 days from  issue date.  Airfare may be subject to increase if fewer than 10 travelers book flights or if Air Request Forms are received

later than April 23, 2010.  Final payment and full legal name of traveler must be submitted by May 7, 2010.
