GROUP REGISTRATION FORM INSTRUCTIONS

An adult representative for each group (regardless of group size) must complete a Group Registration Form.  To reserve your spot now, simply follow the instructions below.

1. Complete the Online Group Registration Form (one form per group).  Please note, this form can be submitted prior to completion of Air Request Form and must be submitted prior to online registration by individuals.

• Complete all information fields. 
• If desired, print a copy for your records.
• Click Submit button at bottom of form.

2. Print, complete and mail Payment Form along with payment. In order to reserve your group's space, a nonrefundable $100 per person deposit must be paid by March 26, 2010.

3. Mail Payment Form, Group Leader Agreement, and payment to:

REGISTRATION DEADLINE: MARCH 26, 2010

Registration will not be processed until the completed

Group Registration Form and deposit check are received.
