GROUP REGISTRATION FORM INSTRUCTIONS
A representative for each group (regardless of group size) must complete a Group Registration Form. For the second year, SADD National will provide subsidies to every SADD student and adult who registers for the SADD National Conference. To reserve your spot now, simply follow the instructions below.
- Complete the Online Group Registration Form (one form per group). Please note, this form can be submitted prior to completion of Air Request and Participation Forms.
- Complete all information fields.
- Print a copy (2 copies if you desire one for your records).
- Click Submit button at bottom of form.
- Attach a nonrefundable deposit check of $100 per person made payable to WorldStrides to the printed copy of the completed form.
- Mail the Group Registration Form and deposit check to:
